Information

For Referring Medical Providers

Typical candidates have to exhaust lower level treatments such as physical therapy, chiropractic care, acupuncture and are not deemed to be immediate surgical candidates. Referrals and requests can be made either for Pain Medicine evaluation by our pain medicine specialist or for the Functional Restoration Program (FRP) evaluation that consists of functional evaluation by a physical therapist, psychological evaluation by a psychologist, and medical evaluation by a medical provider. Once patients are seen by our pain medicine specialist or have had FRP evaluation, we will obtain authorization for the actual program at 2 week increments based on their progress. Patients will return back to their primary treating providers upon completion of the functional restoration program and can be seen by our pain medicine team on as-needed basis in future.

For Insurance Carriers and Employers

Patients are prescreened to have tried and failed lower level treatments under Medical Treatment Utilization Schedule (MTUS) Guidelines. Patients either deemed not to be surgical candidates and have had surgery and are still unable to return back to work and perform activities of daily living. Patients will be deemed to reach the state of the Maximal Medical Improvement (MMI) upon completion of the program. One of the main goals of the program is to enable patients to return back to work and minimize future utilization of medical resources. As a part of our commitment to measurable outcomes, patients who have completed our program are offered future aftercare treatments to help them adjust to returning back to work or to a different employment free of charge to the insurance carrier. Can request participation in weekly meetings for your patient. Weekly meetings with medical staff, biweekly objective goal set. Biweekly team meetups
